Health Care Decisions – An Important Conversation

During the month of April, many healthcare providers and caregivers take the opportunity to recognize the importance of education to empower patients to make their wishes known regarding their health care, and for healthcare providers and caregivers to understand the importance of honoring their choices.

We don’t always know when our own health or the health and well-being of our loved ones could take an unexpected turn. Conversations around advanced care planning and advanced directives are an important way to foster peace of mind, clarify personal goals and ensure dignity and respect for the very personal decisions that we can make.

At Notre Dame Health Care our care teams within our At Home Division (hospice and palliative care) routinely encourage these meaningful conversations and help provide resources to those individuals and families working to formalize their health care decisions. We welcome the opportunity to talk with you about your unique needs, please feel free to contact us at (508) 852-5505 to learn more.

The National Hospice and Palliative Care Organization (NHPCO) offers some wonderful tips and links to resources in a recent article promoting National Healthcare Decisions Day which takes place annually on April 16. No matter what day you may choose to consider these opportunities, taking the time can be comforting and empowering.
